UPDATE: ENV-105 Extends Median PFS to 17.7 Months in mCRPC
We updated you about the positive results of carotuximab (ENV-105) in this previous post. Now phase 2 data suggest that ENV-105 combined with apalutamide may substantially delay progression in metastatic castration-resistant prostate cancer (mCRPC) compared with apalutamide alone.
In the randomised study, median progression-free survival reached 17.7 months with ENV-105 plus apalutamide, compared with just 2 months with apalutamide alone. The result builds on earlier interim findings that had already suggested prolonged disease control and frequent PSA declines in patients receiving the combination.
ENV-105 is an antibody targeting CD105, also known as endoglin, a protein involved in tumour signalling, angiogenesis and treatment resistance. The therapeutic idea is unusual: instead of simply adding another cytotoxic treatment after resistance develops, blocking CD105 may help restore sensitivity to androgen-receptor-directed therapy.
Earlier data from the same programme had shown a median progression-free survival of roughly 13–14 months in a small cohort, together with PSA declines in most evaluable patients. The new 17.7-month figure therefore suggests that the initial signal may be persisting with longer follow-up.
The study is now being expanded to test ENV-105 earlier in the course of resistance. Patients showing PSA progression after an initial androgen-receptor pathway inhibitor will be randomised to enzalutamide alone or enzalutamide plus ENV-105, with the aim of determining whether blocking CD105 can delay or reverse emerging hormonal resistance.
