Pluvicto Moves Into First-Line Metastatic Prostate Cancer, Marking Shift Toward Earlier Radioligand Therapy

A new regulatory approval is reshaping the treatment landscape for metastatic hormone-sensitive prostate cancer (mHSPC), bringing radioligand therapy into much earlier stages of disease. Pluvicto (lutetium Lu-177 vipivotide tetraxetan aka 177Lu-PSMA-617), a therapy targeting prostate-specific membrane antigen (PSMA), can now be used in combination with standard androgen deprivation therapy and androgen receptor pathway inhibitors in patients with PSMA-positive metastatic disease.

The decision is supported by results from the large Phase III PSMAddition trial showing a significant delay in disease progression. Patients receiving the Pluvicto-based combination experienced a 33% reduction in the risk of progression or death compared to standard therapy alone, with a favorable trend in overall survival that continues to mature. These findings reinforce the growing consensus that intensifying treatment earlier in the disease course can improve outcomes.

Metastatic hormone-sensitive prostate cancer remains incurable, and a substantial proportion of patients progress to castration-resistant disease within two years despite current standard treatments. The introduction of a targeted radiotherapy approach at this stage offers a mechanistically distinct option, delivering radiation directly to PSMA-expressing tumor cells while sparing most normal tissue.

Safety findings were consistent with prior experience in later-stage disease, with manageable increases in hematologic and constitutional adverse events. Importantly, overall quality of life was maintained, suggesting that the added clinical benefit does not come at the cost of significant patient burden.

This approval effectively extends PSMA-targeted radioligand therapy across the full spectrum of metastatic prostate cancer, marking a shift toward precision treatment earlier in the disease trajectory. As longer-term survival data become available and access to PSMA imaging expands, this approach is likely to play an increasingly central role in first-line management strategies.
